Q&A

Long on social needs screening and care coordination

New York City Council · Oct 28, 2026 · starts 2:09:09 · 44 sec

Long says about a quarter of NYC Care patients screen positive for a social need like food or housing instability and are routed to care coordination with a community health worker or social worker. He says no patient leaves a visit without a resource.

Dr. Ted Long

So the care that we provide for our NYC care members is identical to the care that we provide for all of our patients.

Dr. Ted Long

That's very, very important to us.

Dr. Ted Long

There's no two levels of care.

Dr. Ted Long

There's one level of care, and it's excellent at New York City Health and Hospitals.

Dr. Ted Long

I think it's about a quarter of our NYC care patients screen positive for having a social need, whether that's food, housing instability, things like that.

Dr. Ted Long

And if you screen positive for that, we put you into our care coordination, basically, program, which can mean it's either a community health worker we're going to assign you to or a social worker or one of our other programs.

Dr. Ted Long

So we treat social needs as a screening process akin to screening you for depression.

Dr. Ted Long

If you screen positive, you go down a pathway, and we make sure that you don't leave the visit without a resource.

Mercedes Narcisse

Thank you.

Dr. Ted Long

Yeah.
