Narcisse asks about other H&H programs; Long highlights SHOW, safety net clinics, and Housing for Health
Narcisse asks about other H&H programs New Yorkers should know about. Long highlights the SHOW street medicine program, safety net clinics that co-locate mental and physical care, and the Housing for Health program, which has housed 1,800 patients.
Beyond New York City Care, are there...
other H&H programs, which talk about how that's just coming to be helping in the process, but
other than H&H programs or services that more New Yorkers should be aware of, particularly those that help improve access to care, preventive services, behavioral health care, or social.
Social support services.
Yeah, there are a couple I would love to use this as an opportunity to highlight.
One is actually both of them are related to some of our most vulnerable New Yorkers, those experiencing homelessness, especially unsheltered homelessness.
So I would love for more New Yorkers to know we have a very robust and large street medicine program, our show, Street Health Outreach and Wellness Program.
We post on our website where we're going to be on a given day.
So we'd love to get the word out to have more people come to us for low barrier medical and behavioral health care right there on the street where they are.
Meeting people where they are is the name of the game with the show program.
We have special clinics, too, at health and hospitals called our safety net clinics, which I'd love more people to know about because if somebody has concerns about...
Can I get my mental health care for severe mental illness along with addressing the wound in my leg?
We have special clinics called safety net clinics where all those services are co-located.
Very impactful.
We've shown to cut the rate of emergency department and inpatient visits down by about half if you go to the clinic.
So very, very impactful clinics.
And then the other thing I'd love for more people to know about is at H&H,
We really care about what happens to you when you leave our clinic.
And we have a program called Housing for Health, which I wanted to take a moment to build up here.
Did you know that at Health and Hospitals, we've now housed 1,800 of our patients?
That's 1,800 patients that if we hadn't done that, if we didn't have this special team, would still be experiencing homelessness.
And now they're in their own home.
And part of the way we've been able to do that, which we'd always love more support on, is we've built 400 housing units on our health and hospitals land.
So we'd love to do more of that or to partner in other ways to get our patients housing.
But we think housing for health is so impactful because if you can combine together,
the primary care you can get at H&H with a home of your own, that I can't think of anything more life-changing than those two things.
